Congressional Death Threats Are Bi-Partisan – Please Stop the Political Show And Get In Touch With Reality

Desperate Hospitals Returns For 2010

<p>I did a series last year called “Desperate Hospitals” and now it seems it’s time once again, unfortunately re-open the series again.  Hospitals have fixed costs and <a href="http://ducknetweb.blogspot.com/2008/08/desperate-hospitals-century-city.html"><img align="right" border="0" height="316" src="http://lh5.google.com/DucknetServices/R7zqj2lNEkI/AAAAAAAADGk/oGkA0r0fnNw/image_thumb[1]?imgmax=800" style="border-right-width: 0px; display: inline; border-top-width: 0px; border-bottom-width: 0px; margin-left: 0px; border-left-width: 0px; margin-right: 0px" width="229" /></a>have pretty much cut down the the bone where they can.  Some hospitals have been lucky with philanthropy kicking in and helping out with money and others have not.  Sometimes this is the big difference along with how they operate.   I stopped with my last update in March of 2009 and here it is a year later almost exactly and here we go again unfortunately.</p> <h4><a href="http://ducknetweb.blogspot.com/2010/01/hospital-lay-offs-will-hit-all-time.html"><font size="2">Hospital Lay Offs Will Hit an All Time High for 2009 – AMA</font></a></h4> <p>I brought back my <a href="http://ducknetweb.blogspot.com/2008/08/desperate-hospitals-century-city.html">“poster child” hospital</a> picture here as well, which was Century City Hospital that closed due to lack of funds, which sits right on the outskirts of Beverly Hills and is across the street from many big Fortune 500 companies located in Century City, where the money’s at.   This somewhat relates to a bit of greed I think when it comes to healthcare versus making a buck.  <strong>In addition if you do a search you will find many hospitals in Canada also laying off workers.</strong>  Please feel free to add more in the comments as I know I didn’t catch all of them.</p> <p><strong>What’s at the base of a lot of this:  Wall Street Algorithms that smashed economy with “programmed desired financial numbers” shown as justification for trading and investments,</strong> while the whole time worked on the back side to create profit centers in areas where they should not have existed. <strong>If you read this blog enough you might just come to hating the word algorithm by now,</strong> as it is the crux of the matter as that is what displays all the decision making data on the screen that everybody uses and hardly anybody questions as the data is the machine gun and we are still running around with swords.  “Tech denial” and lack of education and the ability to fight back takes your money in a flash and I sure hope we have a Congress that soon will come to terms with this.  BD   </p> <p><u>Here’s what is on the web today:</u></p> <p><a href="http://online.wsj.com/article/SB20001424052748703409804575144184025495398.html"><strong>In New York City</strong></a></p> <p>NEW YORK—The nation’s largest public hospital system plans to slash its work force—including doctors and nurses—by about 10% over two years as government aid drops and the number of uninsured patients jumps. </p> <p>With its budget deficit set to top $1 billion, New York City’s Health and Hospitals Corp. plans to eliminate 2,600 jobs in the fiscal year that begins July 1. Now, its operators are taking another step, and we're learning details about the hundreds of layoffs.  The hospital has been on shaky financial footing for years, according to employees with whom we spoke. A Case of “Dirty” Algorithms – 2 Madoff Computer Administrators are Indicted – Illegal Coding and Networking for Big Profits

<p><strong>This is worth mentioning as this is not by far the only case where illegal IT operations are happening.</strong>  With writing code and algorithms, programmers can adjust to <img align="right" border="0" src="http://lh4.ggpht.com/_v3zjJigoAPE/S20T_wMTeTI/AAAAAAAAcZE/lmCuiFEtDMA/image_thumb[1].png?imgmax=800" style="border-bottom: 0px; border-left: 0px; display: inline; margin-left: 0px; border-top: 0px; margin-right: 0px; border-right: 0px" />reflect some “desired” numbers.  This is the base to where it all begins and takes place and illusions are created.  <strong>We need to have our government IT structure and talent up to this level in order to find and catch the wrong doers.</strong>  This did not happen for years as we were <strong>living in a world of “bliss” to where politicians lived in a world of “tech denial” and the coders just kept on working as they had a huge envelope here for fraud as they knew they had the machine guns and everyone kept running around with swords.</strong>   </p> <p><strong>All coding and networking is not created equal and like any other area of life, this needs to be audited,</strong> but again we didn’t have enough individuals who embraced that this was happening and kept living in a world of bliss and using outdated methods of tracking crime, which as well all know didn’t work.  <strong>Here’s an example below where an insurance company was running “HIV” algorithms to find affected patients.</strong>  When such code is run the screen presents data and then it’s up to the minds of those who queried the information as to what choice of action they will take.  These folks were caught, but I am guessing there’s much more going on.</p> <h4><a href="http://ducknetweb.blogspot.com/2010/03/health-insurance-company-ran-hiv.html"><font size="2">Health Insurance Company Ran an “HIV” Algorithm To Cancel Consumer’s Policy –An Automatic Fraud Investigation Revealing “False Positives” Or “Unhealthy Patients”</font></a></h4> <p>We are really behind here with members of Congress coming to this conclusion and live in denial and still try to use old methodologies for laws and rules.  A good example is this link below, it has taken them all this time to come up with a law to monitor peer to peer use by government employees?  Shoot every business has long been all over this for years.  </p> <h6><a href="http://ducknetweb.blogspot.com/2010/03/breaking-news-house-passed-bill-to.html"><font size="2">Breaking News: House Passed a Bill to Prevent Government Employees From Using Peer to Peer File Sharing!</font></a></h6> <p><strong>So when our government leaders talk about monitoring health insurance companies, they need to have the infrastructure and technology to do so and not just make a law that is not enforceable.</strong>  This is not to say that our leaders need to learn code, but they need to understand how this drives the economy and the laws we live by today.</p> <h6><a href="http://ducknetweb.blogspot.com/2009/08/department-of-algorithms-do-we-need-one.html"><font size="2">“Department of Algorithms – Do We Need One of These to Regulate Upcoming Laws?</font></a><img align="right" border="0" src="http://lh5.ggpht.com/_v3zjJigoAPE/S6eaYa9I57I/AAAAAAAAc-0/f0wK8itMuwc/image_thumb[2].png?imgmax=800" style="border-bottom: 0px; border-left: 0px; display: inline; margin-left: 0px; border-top: 0px; margin-right: 0px; border-right: 0px" /> </h6> <p><strong>If we don’t wise and read up at leadership levels, we are basically screwed and with transparency being what it is today, more “stupid” and “ignorant” schemes for political and monetary gain will keep appearing in the news.</strong>  We have a full on leadership of “non participants” and that will sink all of us faster than what we can shake a stick at.  Look at the Madoff example and see how long “bliss” and “ignorance” prevailed here, and we all know that story now.  <strong>In healthcare, we give anti fraud contracts to the same entities that used code to generate profits that were upheld in court as being “illegal”.</strong>  Again, this tells me there’s nobody home when it comes to auditing and ensuring our safety as citizens.  I urge everyone to take a few minutes every day to read up and learn something and read the news.  </p> <p><strong>Technology works behind the scenes and if you are not aware of what is happening, bliss and ignorance takes all your money in time.</strong>  In reading this post I see where it is noted that the<strong> SEC reviewed the books 5 times, and nothing came of it, programmers doing their hired job and wrote some pretty good deceptive code to keep the wolves away.</strong>  Hopefully now justice will be done.  Everything is not as it seems today with the layers of technology and intelligence we have available and with leaders who live in the world of bliss it scares me that this is eventually going to do us in and we will no longer be the world leader as other countries are moving quickly above our current status today.  BD  </p> <blockquote> <p>A <a href="http://www.justice.gov/usao/nys/pressreleases/March10/oharajeromeandperezgeorgeindictmentpr.pdf">statement</a> released yesterday by the U.S. Attorney's office for the Southern District of New York said the two men started working for Bernard L. Almost 25% of Israelis Can’t Afford Healthcare – Survey

Breaking News: House Passed a Bill to Prevent Government Employees From Using Peer to Peer File Sharing!

<p><strong>I say this is breaking news as almost everyone else in business has a policy and has had for quite some time!</strong>  I did put it on Twitter but felt later it deserved it’s <a href="http://lh3.ggpht.com/_v3zjJigoAPE/S6vwUayitXI/AAAAAAAAdCc/HpyrOGQw7BE/s1600-h/image%5B9%5D.png"><img align="right" alt="image" border="0" height="240" src="http://lh4.ggpht.com/_v3zjJigoAPE/S6vwVfFbB_I/AAAAAAAAdCg/nC_j1oxCuWA/image_thumb%5B5%5D.png?imgmax=800" style="border-right-width: 0px; display: inline; border-top-width: 0px; border-bottom-width: 0px; margin-left: 0px; border-left-width: 0px; margin-right: 0px" title="image" width="214" /></a>own post.  I have gone into physician offices and hospitals and removed the peer to peer software where it was installed when coming in to consult.  One office had some really personal stuff from the doctor shared throughout the office!  That was embarrassing as most do not understand how to configure the software and it by default would share the document files.   </p> <h4><a href="http://ducknetweb.blogspot.com/2010/03/file-sharing-peer-to-peer-programs.html"><font size="2">File Sharing Peer to Peer Programs Still Posing Risk to Exposing Patient Data – Think Twice About This</font></a> </h4> <p><strong>These folks are the same ones that are concerned about data security and just passed a huge Healthcare Reform package right?</strong>  This worries me.  What this says is that we have a bit of lack of education in software that almost every kid wants to use to get free music, videos and other items from connecting to another computer on the web, but yet we have leaders that have no clue what in the heck this is, been around for years and Napster was the beginning years ago. <strong>We have folks that don’t pay attention as to how technology is shaping our world.  Is there really a true understanding in Congress as to what HIPAA is all about?</strong></p> <p>Who lets this stuff get through the networks?  They must have some really good IT folks who firewall the network, either that or nobody is aware.  Look at this story that happened at a hospital with a worker infecting a computer.  </p> <h6><a href="http://ducknetweb.blogspot.com/2009/09/man-accidentally-infects-ohio-hospital.html"><font size="2">Man Accidentally Infects Ohio Hospital with Spyware – Was Meant for Girlfriend’s Home Computer and She Opened Her Yahoo Mail at Work Instead</font></a></h6> <p><strong>At least somebody had the sense to use Tiversa,</strong> software that finds information on peer to peer networks that should not be on the web.  That’s the defense that has to be used for the dummies that share everything out there who can’t or don’t know how to configure the software.   </p> <h4><a href="http://ducknetweb.blogspot.com/2009/02/peer-to-peer-forensic-services.html"><font size="2">Peer to Peer Forensic Services – Enterprise solutions for Hospitals and other Health Agencies</font></a></h4> <p>Again, this just somewhat floors me that this is just now being looked at. <strong>The Senate wants alerts to notify individuals that peer to peer software is being used</strong> – not good enough.  Again when I read this it just makes me shudder at the thought of how little is know perhaps about security by those making the laws for this country.  There are business program now that use Peer to Peer, but network administrators set up security and firewalls for those ports to use used only, so here I am talking about recreational peer to peer sharing.  </p> <h4><a href="http://ducknetweb.blogspot.com/2009/02/healthcare-workers-sharing-music-and.html"><font size="2">Healthcare Workers Sharing Music and they could also be sharing Medical Records and Files</font></a> </h4> <p>Again, you would have thought this would have been addressed way back when Napster was going strong.  Also one other note that I found interesting and posted about <strong>last year was the big ruckus over Representatives using Twitter?  Twitter is nothing with 140 text characters compared to this but it took an Act of Congress to allow and yet peer to peer lives on.  Go figure!!</strong></p> <h4><a href="http://ducknetweb.blogspot.com/2008/07/in-house-tweets-fly-over-web-plan.html"><font size="2">In House, Tweets Fly Over Web Plan – Washington</font></a></h4> <p>Here’s a short excerpt from the Twitter/QIK incident, funny now when you think about Congress is just getting around to the real dangerous stuff with exposing confidential information.  Twitter by comparison is harmless.  </p> <p><em><font size="1">“I just learned the Dems are trying to censor Congressmen’s ability to use Twitter Qik </font></em><a href="http://topics.nytimes.com/top/news/business/companies/youtube/index.html?inline=nyt-org"><em><font size="1">YouTube</font></em></a><em><font size="1"> Utterz etc — outrageous and I will fight them,” Representative John Culberson, Republican of Texas, wrote last Tuesday on his personal page on the online text-messaging site Twitter, where he posts a daily, rapid-fire log of his thoughts. In 2009, as the nation struggled through a recession, people relied on library technology to find work, apply for college, secure government benefits, learn about critical medical treatments, and connect with their communities.  </p> <p>The report, <i>Opportunity for All: How the American Public Benefits from Internet Access at U.S. Libraries</i>, is based on the first, large-scale study of who uses public computers and Internet access in public <a href="http://www.gatesfoundation.org/libraries/Pages/united-states-libraries.aspx">libraries</a>, the ways library patrons use this free technology service, why they use it, and how it affects their lives. It was conducted by the University of Washington Information School and funded by the Bill & Melinda Gates Foundation and the Institute of Museum and Library Services. </p> <p><strong>37 percent focused on health issues. The vast majority of these users (82 percent) logged on to learn about a disease, illness, or medical condition. One-third of these users sought out doctors or health care providers. Biometric Monitoring to be Used by Florida HMO to Combat Healthcare Fraud – BioClaim Linking CMS 1500 To The Patient Visit

<p>The Biometric log in device replaces the usual and standard paper sign in sheet we have all used for years.  The fingerprint is converted to a unique data code and<a href="http://www.bioclaim.com/The-Solution/"><img align="right" alt="image" border="0" height="64" src="http://lh5.ggpht.com/_v3zjJigoAPE/S6uvnhrTF9I/AAAAAAAAdCM/4fRfmYsdYN8/image%5B1%5D.png?imgmax=800" style="border-bottom: 0px; border-left: 0px; display: inline; margin-left: 0px; border-top: 0px; margin-right: 0px; border-right: 0px" title="image" width="202" /></a> not stored itself.  This is the same system being used at Disney in Florida.</p> <p>75% of fraud is stated to be done by the provider as called “phantom billing”.  The 1500 claim form will link and provide the finger print to show that the patient actually showed up.  The software process will run before every claim is submitted.  </p> <p>The first visit enrolls the finger print and given the unique ID number to include the date, time and location and the template is stored at the BioClaim central data base.  One item this adds is the fact that billing might be done faster.  <strong>No finger print, no claim is filed.</strong>  This will keep patients from trying to use another person’s insurance card.  A claim would be denied immediately if the correct person was not linked.  I would guess there might need to be some type of over ride added for an telehealth visit though as well if the practice is enrolled in one of those services, but individual software might have that taken care of to bypass.  Basically the software is identifying the patient correctly and not allowing for any items to be billed where a fingerprint for the visit is not available.  I have seen biometric log ins but this is somewhat the first I have seen combining check in with claim submissions.  BD  </p> <p><strong><u>From the website:</u></strong></p> <p><em><strong>“Phantom Billing</strong> - This is billing for services that were not provided, billing for services to persons accompanying a patient who did not receive any medical <a href="http://lh3.ggpht.com/_v3zjJigoAPE/S6uvQvWd-vI/AAAAAAAAdB0/MBeGIzrAOBU/s1600-h/image%5B18%5D.png"><img align="right" alt="image" border="0" height="139" src="http://lh3.ggpht.com/_v3zjJigoAPE/S6uvRM6r-zI/AAAAAAAAdB4/Jvw8aHNyVFQ/image_thumb%5B12%5D.png?imgmax=800" style="border-right-width: 0px; display: inline; border-top-width: 0px; border-bottom-width: 0px; margin-left: 0px; border-left-width: 0px; margin-right: 0px" title="image" width="184" /></a>services, or billing for office visits that never occurred.</em></p> <p><em><strong>Card Swapping</strong> - This occurs when a patient receives treatments using another person's medical information.</em></p> <p><em><strong>Up-coding</strong> - This involves billing for more expensive treatments that services actually provided during a patient's visit.</em></p> <p><em><strong>Medical Identity Theft</strong> - This involves someone pretending to be someone else by using a person's name or other items such as an insurance card or number to obtain medical services. Medical identity theft can result in incorrect medical records being stored under the victim's name and insurance information.</em></p> <p><em>Each of the above items are problems that result in a larger price tag for everyone; payer, provider and patient. Currently fraud detection is mostly done using computerized statistical analysis or by persons in the industry recognizing and reporting suspicious claims. This is a "pay and chase" methodology, pay the claim then, when fraud is detected, the fraudulent claims must be investigated further to provide proof that the claims are in fact fraudulent and then they can be pursued further. The Unintended? Consequences of Healthcare Reform – Opinion From Dr. Crounse at Microsoft

<p> I think Dr. Crounse has some pretty good observations on where healthcare is headed and the word “change” is upon us.  He reiterates on the use of technology as a <a href="http://www.microsoft.com/industry/healthcare/healthtechtoday/default.aspx"><img align="right" alt="image" border="0" height="86" src="http://lh4.ggpht.com/_v3zjJigoAPE/SvR8vzjoiKI/AAAAAAAAa2k/JuzvAdDlWOM/image%5B13%5D.png?imgmax=800" title="image" width="188" /></a>means of survival, which is nothing new and many including myself have said the same thing in perhaps different words.  Below are a few of the highlights from his post.  Also mentioned is the growth of e-Health with the use of telehealth, kiosks, and telemedicine.  If you regularly read his column and watch the monthly series he produces called <a href="http://ducknetweb.blogspot.com/2009/11/health-tech-today-with-dr-bill-crounse.html">“Health Tech Today”</a> you can see some of the latest technology being used both here in the US and in other countries.</p> <p>One other point made and I have covered a lot of the same type of articles here is the mention of <strong>who is providing the venture capital for many of the eHealth ventures, it is the insurance companies.</strong>  The link below is one example and today United Healthcare is generating more profits from their “technology” areas than they do selling policies.  Those orange tablets from Phreesia that you may see being used at your doctor’s office are one example of where venture capital money from Blue Cross/Blue Shield goes.  </p> <h4><a href="http://ducknetweb.blogspot.com/2008/11/bluecross-blueshield-create-new-venture.html"><font size="2">BlueCross BlueShield Create New Venture Capital Organization</font></a></h4> <p>Basically the cost of preventive and normal physician office visits is being shifted back to the patient and a good reason to get a personal health record started as providers are becoming more mobile than ever with working in more than one location sometimes.  As Dr. Crounse suggests, if you don’t or can’t pay more than what is allowed, chances are you will be seeing a nurse and not a doctor.  <strong>Technology is impacting every area of our lives and the use of software for better health decisions is growing,</strong> and if no other good reason can come to mind,  it can’t reside in our heads or on reams of paper as we might either forget or spend wasted hours looking for information when a computer and software does it in a few seconds.  <strong>Healthcare needs business intelligence analytics</strong> and I remember just 3-4 years ago bringing up this conversation with some local hospitals and they thought I was crazy, and when you look today it’s everywhere and part of the system of survival.   </p> <p>My own personal opinion on how we ended up here goes back to the creation of Medicare Part D arrived from <strong>completely outsourcing without the US government years back updating and creating their own infrastructure</strong>, so now we have no back up within the government at present to facilitate running a similar program for benefits and care, although it’s never too late to start planning and creating one. <strong> A good “tech geek” will always reiterate the need for back up</strong>, no matter where you are in the IT business, and to me this is especially true with Health IT.  There will always be a mix of both government and private infrastructures and the key for the success is balance and I hope someday we can reach the ultimate balance that works for all.  </p> <p>I do ponder what would have happened if Congress would have created a “private” or “public” plan; border-left: 0px; display: inline; margin-left: 0px; border-top: 0px; margin-right: 0px; border-right: 0px" title="image" width="362" /></a>think these numbers are exaggerated, I’ve noticed a surprising increase in email traffic from docs asking about jobs at Microsoft or wanting to pick my brain about alternatives to patient care.</strong>  Usually these are primary care physicians who have experienced a significant drop in income the past few years, but I’m perplexed by the number of specialists who’ve also contacted me.  <strong>Most pundits agree that the future of primary care is likely to fall into the hands of physician extenders such as nurses and nurse practitioners.</strong>  That’s probably OK since very few people will be dedicated enough to endure 7 or 8 years of very expensive, post graduate education for a wage that barely breaks six figures.</em></p> <p><em><strong>If you really want a primary care physician, be prepared to join a concierge practice or at the very least get your medical care from a large multi-specialty clinic where primary care is subsidized by specialists.</strong>  I expect to see a robust climate for a wide variety of concierge, monthly membership fee structured practices like </em><a href="http://md2.com/index.php"><em>MD2</em></a><em>, </em><a href="http://www.mdvip.com"><em>MDVIP</em></a><em>, </em><a href="http://hellohealth.com/"><em>HelloHealth</em></a><em>, etc.  If you are unwilling to pay more than your health plan or the government will provide, then <strong>you had better get used to seeing a nurse.</strong>  Or, you may decide to get your primary care from a retail clinic in your local pharmacy or big box store.  <u>Either way, you’ll still be seeing a nurse.</u></em></p> <p><em>At Microsoft we are working with a number of hospitals and healthcare provider organizations around the world on solutions that help them manage the patient experience end to end.   <strong>New rules penalize hospitals for patients that get readmitted within 30 days of discharge.</strong>  There is a huge incentive to keep these people out of the hospital by doing a better job of monitoring them at home or elsewhere and intervening before they end up back in the emergency room.  </em><a href="http://www.microsoft.com/industry/government/solutions/health_chronic_conditions/default.aspx"><em>Chronic condition management</em></a><em>  solutions rely on <strong>powerful data analytics</strong> that can help predict who is most at risk for readmission, and also on<strong> relationship management software that follows the patient from discharge to home and maintains regular, often automated contact for appointment reminders, medication prompts, symptom surveys, and patient and family education</strong>.”</em></p> </blockquote> <p><a href="http://blogs.msdn.com/healthblog/archive/2010/03/24/the-unintended-consequences-of-healthcare-reform.aspx">HealthBlog : The Unintended?
