Yes. When you obtain an ABN for a service you perform that you don't expect Medicare to pay for, report the service with modifier –GA (waiver of liability statement on file).
Modifier –GA tells Medicare that the beneficiary signed a valid ABN and that he understands he's responsible for payment of the specific, stated service or procedure you provided if Medicare doesn't cover it. Without a valid ABN, a Medicare patient can't be held responsible for the charges Medicare won't cover."

Use a code from the 99341-99345 series for home services provided to a new patient or a code from the 99347-99350 series for a home visit to an established patient. Remember to use 12 as the place of service code.
