EHRs need Standard Templates – So Let’s Look at the Common User Interface Project, a lot of the work is already in progress and partially completed

<p> If you have read this blog long enough, there are multiple posts about The Common User Interface, an open source project from Microsoft that would in fact have the capabilities to present the same set of templates, data entry screens, etc. at all hospitals and physician offices.  The NHS in the UK has been the pioneering force in bringing the project to light; however there have been thousands of downloads from the site, so there are developers working all over to hopefully bring this to light.  <img align="right" src="http://lh4.ggpht.com/DucknetServices/SCmrYmUInpI/AAAAAAAAGqA/e2Zfau-nbuk/image_thumb7.png?imgmax=800" style="display: inline; margin-left: 0px; margin-right: 0px" /> </p> <p>I don’t know of any physician or medical staff member who would not like to see the same interface at any hospital or doctor’s office, as how many different software applications do they have time to learn today and still provide high quality healthcare.  Unfortunately just due to the way medical records have evolved with different companies creating their own software solutions we have a myriad of software applications that are getting more confusing and difficult for the end users all the time, and this really is not just limited to healthcare for that matter.</p> <p>The project is dynamic and includes some very nice Silverlight effects as well, a real potential winner in my estimation, and the pictures below show how it looks in it’s present form.  The related reading has videos and even more additional information to read to learn about the project and how the interface would serve to bring everything together under one roof, and this is open source, so anyone can download and develop, and Linux for Silverlight is out there now too, so it is multiplatform. <strong> So would this not be nice to have the same screens whether or not the hospital</strong> used Microsoft Amalga, McKesson, Cerner, Epic, Eclipsys, etc. just to name a few, to have the same or similar interface at all hospitals, something HHS might want to take a look at when considering common templates and user interfaces maybe, why start at this point and try to reinvent the wheel for goodness sake when so much of the code needed is already done and more is on the way with developers.  BD  </p> <p>  <img src="http://lh5.ggpht.com/DucknetServices/SOuW8CfhrdI/AAAAAAAAJX4/HkLRVrmnr_k/image_thumb5.png?imgmax=800" /></p> <p><img height="311" src="http://lh4.ggpht.com/DucknetServices/SOEcT38TB-I/AAAAAAAAJMY/lTG8UH1utgU/image[9].png?imgmax=800" width="402" /> </p> <p><img height="327" src="http://lh5.ggpht.com/DucknetServices/SCmrW2UInnI/AAAAAAAAGpw/69_cKKhEsWQ/image22.png?imgmax=800" width="408" /> </p> <p><img height="230" src="http://lh3.ggpht.com/DucknetServices/SOuXSAufC9I/AAAAAAAAJYo/P7wKCqURmu0/image_thumb16.png?imgmax=800" width="403" /> </p> <p><img height="232" src="http://lh6.ggpht.com/DucknetServices/SOuXZp6wpII/AAAAAAAAJY4/gQ1AlxAi0tA/image_thumb29.png?imgmax=800" width="406" /> </p> <blockquote>The Department of Health and Human Services should support creation of standardized, structured templates to ease the collection of regulatory data via electronic health records, according to an advisory group. Innovation Without Collaboration Is Fouling Up The US Healthcare IT System–We Need Both As We Can’t Stand on Innovation Alone

<p>Every week I read in the news “we need more innovation” and for goodness sakes <u>in Health IT we have innovation running out our ears!</u>  Everyone is so busy building the better mouse trap that <u>we have forgotten how to collaborate and work together.  </u>We are seeing some signs of this and<u> it usually comes in the form of a merger or acquisition as the economy is dictating much of that to figure out how to maintain current profit levels or how to gain more.</u>  <a href="http://lh6.ggpht.com/_v3zjJigoAPE/TMdlzzhTk2I/AAAAAAAAibE/kswet5nTyDs/s1600-h/image%5B16%5D.png"><img align="right" alt="image" border="0" height="183" src="http://lh4.ggpht.com/_v3zjJigoAPE/TMdl0mOm81I/AAAAAAAAibI/ShbQzkEcDAY/image_thumb%5B6%5D.png?imgmax=800" style="background-image: none; border-right-width: 0px; padding-left: 0px; padding-right: 0px; display: inline; float: right; border-top-width: 0px; border-bottom-width: 0px; border-left-width: 0px; padding-top: 0px" title="image" width="216" /></a></p> <p>We do a pretty poor job on collaborating and yes there’s a certain amount of competition needed, but in Health IT business intelligence with added transaction fees for each algorithmic process that is added, it is eating us up alive.  <u>In doing this blog I see new press releases every day and what is occurring is all are moving to where the money is, the payables side.</u>  Healthcare reform is working to try and eliminate some of this but in working with old paradigms we keep running into a dead end and the costs for this just keep on rising.</p> <p><strong>There are algorithms written for “accurate” results and those written for “desired” results and the 2 are not always the same and thus when purchasing software or web based services it’s easy to get confused as you will usually get what you bought but at what cost?  </strong></p> <p>We have just about drained one side being the hospitals and doctors that provide these services, <strong>but the payer side with additional complicated algorithms keeps growing and growing and for the most part this is all proprietary systems generated to make a profit.</strong>  I don’t deny anyone the opportunity to make money but the scale is lopsided to the extreme to where it ties one hand behind the clinical side today.  Pharma and insurance companies are not going to be able to sustain the levels of <a href="http://lh3.ggpht.com/_v3zjJigoAPE/TMdl09BEamI/AAAAAAAAibM/9OAS1lx_uH4/s1600-h/image%5B11%5D.png"><img align="right" alt="image" border="0" height="62" src="http://lh4.ggpht.com/_v3zjJigoAPE/TMdl1Nl45mI/AAAAAAAAibQ/IchjYH91ETU/image_thumb%5B4%5D.png?imgmax=800" style="background-image: none; border-right-width: 0px; padding-left: 0px; padding-right: 0px; display: inline; float: right; border-top-width: 0px; border-bottom-width: 0px; border-left-width: 0px; padding-top: 0px" title="image" width="204" /></a>profits seen in the past, it’s a given and to keep pulling that cord and having drugs covered with extensive patents and costs won’t cut it, same with insurance and those folks are the experts on how to adjust algorithms for profit, they live by them. </p> <p>This is worth a mention here and I have repotted on it many times with the<strong><u> open </u></strong><a href="http://www.mscui.net/"><strong><u><img align="right" alt="image" border="0" height="277" src="http://lh6.ggpht.com/_v3zjJigoAPE/TMdl1kc4hRI/AAAAAAAAibU/Vz1N4qwkTVM/image%5B12%5D.png?imgmax=800" style="background-image: none; Electronic Health Records Not Enough – Aggregation and Better Interfaces Needed

Genomics and Silverlight from Microsoft working together for Great Visual Enhancement– BioMashUps

<p><strong><u>Hard Hat Area:</u></strong>  The University is soon to post on <strong>CodePlex,</strong> the open source community page from Microsoft.  What is amazing to me is the fact that the Popfly platform was used for Silvermap, so now we have bio-mashups too.  The processing of images can now take place at the desktop instead of the server level and this speeds up the process tremendously instead of having to wait.  The amount of information to be read and deciphered is huge and this brings the desired information into a form where it can be digested and utilized quickly.  </p> <p>BioMashups are defined here:  </p> <h5><strong><a href="http://www.mquter.qut.edu.au/bio/biomashups.aspx">Biomashups and Mashup Frameworks</a></strong></h5> <p><em>“An alternative approach to bioinformatics analysis is to make use of web mashups. Common User Interface – EHR Development Work in Progress

<p> Recently I posted about the <strong>Common User Interface from Microsoft</strong> and I spoke with a developer in <strong>New Zealand, Synapse,</strong> who has done some integration work with the interface.  Graham Chiu of Synapse, is a physician as well as a developer so  he is one busy person and nice to see a physician to be able to find the time for software development.  Below I have screenshots of both the <a href="http://synapsedirect.com/products/default.aspx">Synapse dashboard</a> and the Common User Interface to show how it looks in Synapse by comparison.<a href="http://synapsedirect.com/products/default.aspx"><img align="right" alt="image" border="0" height="53" src="http://lh6.ggpht.com/DucknetServices/SOuWzBAqFWI/AAAAAAAAJXo/1XsS4x_eRfU/image%5B5%5D.png?imgmax=800" style="border-right: 0px; EHR Vendors to Focus on Usability? What a Surprise, Many Systems Have Done This For Years, EHR Systems Owned By Insurers Though Might Be Subject to Different Flavors and Goals

<p>I go back to the pre ONC days when I was developing.  <strong>Did I focus on usability, you bet I did.  In those days everything had not yet <img align="right" src="http://lh5.ggpht.com/DucknetServices/SCmrW2UInnI/AAAAAAAAGpw/69_cKKhEsWQ/image22.png?imgmax=800" style="display: inline; I was impressed as Dr. Crounse knew me as the “Medical Quack”, one huge compliment for me in knowing where this blog reaches at times. </p> <h4><a at the show (with my tablet pc) I had an <strong><a href="http://ducknetweb.blogspot.com/2008/03/electronic-medical-records-of-future.html">old demo version from Microsoft running on the tablet,</a></strong> and this was just before the public release on the Common User Interface project, and it drew a lot of attention.  I still keep seeing this YouTube video I made still popping up occasionally on websites today.  <strong><a href="http://ducknetweb.blogspot.com/2008/03/tabletkiosk-i440d-sahara-tablet-video.html">I even got a little carried away</a></strong> with doing a speech recognition demo on the floor, which of course was not perfect, but rather just showing another way of interacting with a computer for input of medical records.  </p> <p>Back to the discussion at hand with medical records, one other thoughts that I have too on the subject is that it is a very complicated situation today.  As software algorithms continue to grow in complexity, so does the software, and that is everywhere we turn today.  This is rapidly presenting a need for standards and simplicity of use for the healthcare industry.  <a href="http://www.codeplex.com/"><img align="right" margin-left: 0px; border-top: 0px; margin-right: 0px; border-right: 0px" title="image" width="163" /></a> </p> <p>Cost of course is a big ticket today with the state of our economy and something that can’t be overlooked, but where<strong> software developers can join force is to create a simpler, yet dynamic method for clinicians to use.</strong>  <strong><a href="http://www.codeplex.com/">Code Plex is a portion of Microsoft that is free and donated open source code</a></strong> available to any developer to work with and within Code Plex there is what is called <a href="http://ducknetweb.blogspot.com/2008/11/common-user-interface-update-and-walk.html">“A Common User Interface”</a> that any software company can download and develop. </p> <p><strong>So what is so important about standards and a common user interface – let me put this bluntly, it’s called ease of use and functionality.</strong>  Let’s take Dr. Smith who is on staff at 3 hospitals, each of them using a different medical records software system. <strong>Did I get your attention yet?</strong></p> <p>Well as we all complain about healthcare being so slow to adapt medical records, this one scores one big negativity factor for the doctors, does this doctor, who’s primary job is to take care of us, <strong>need to learn 3 different software systems,</strong> and how about the time it takes him.  Ok, we all go through software updates everywhere we go today, but keep in mind all the information about each patient that is flowing today to a physician.  <strong>It’s an administrative nightmare for the physician, no matter how you look at it and it’s no wonder the frustration and non-participation levels are high, put yourself in those shoes.</strong>   I have done my own little survey in asking doctors how they would feel if the screens and data entry were all the same at any hospital they happen to be on staff at or have a working relationship,<strong> do you think any of them said “no” I wouldn’t like that, of course not.</strong>  <u> I think one would have to be a bit crazy to say I like having 3 different input screens to learn, agree? </u></p> <p>So what we are talking about here is not only medical records, but a system that can work and have a familiar face at any facility.  The <strong><a href="http://ducknetweb.blogspot.com/2008/11/common-user-interface-update-and-walk.html">Common User Interface</a>,</strong> as being developed by the NHS is something that should not be overlooked as much of the work is already done!  Do we really need to re-invent the wheel?   It does make me somewhat wonder if software developers are trying to do just that when reading articles such as this one, href="http://ducknetweb.blogspot.com/2008/11/common-user-interface-update-and-walk.html">Common User Interface,</a></strong> again free for taking and developing to perhaps one day bring the same dynamic data entry screens and processes to every hospital and physician’s office across the US.  Years ago I wrote a very simple EMR before the complexities of today and have followed the progress and am an active contributor and moderator on <strong><a href="http://www.emrupdate.com/forums/">EMR Update, a</a></strong> site discussing electronic medical records.  The physicians at <strong><a href="http://www.emrupdate.com/forums/">EMR Update</a></strong> came to know me around there a few years ago and we have a great exchange of technical and clinical information where input from both sides can lead to solutions and you will also find this blog syndicated there as well.  </p> <p><a href="http://ducknetweb.blogspot.com/2008/11/ehrs-need-standard-templates-so-lets.html"><img align="right" src="http://lh5.ggpht.com/DucknetServices/SOuW8CfhrdI/AAAAAAAAJX4/HkLRVrmnr_k/image_thumb5.png?imgmax=800" style="display: inline; margin-left: 0px; margin-right: 0px" /></a></p> <p><strong>Dr. Crounse on his blog also has a nice <a Interface with</a> links to the MSDN technical pages.</strong>  When you stop and think about it, the interface makes sense and I wonder where the delay is myself with developers who might not be taking a serious look at coming together to make the process easier, you have to think of the end user today with simplified software screens, otherwise it’s just another overloaded software system.<strong> Me, being a consultant too, just from my tiny corner of the word, how many software systems do I have to learn to be able to give support?  I feel the same as the doctors on this one.  (grin).</strong></p> <p>It just makes one wonder why perhaps we might not be a little more united in this effort to create a product that would have simplicity, dynamics, and something that would only
